Who We Are
The NeuroCenter offers complete diagnostic and treatment services for neurological conditions. Since 1981, thousands of patients have benefited from the Neurocenter’s services. The wide spectrum of tests available at the NeuroCenter minimizes the inconvenience of having to visit multiple testing and treatment centers. The NeuroCenter's warm, relaxed, friendly atmosphere provides a sense of well-being from your very first visit.
The initial history, examination and subsequent visits are conducted in their entirety by a specialist who will be primarily responsible for your treatment. The same specialist orders and assesses the necessary tests and coordinates the evaluation by other specialists when appropriate.
The NeuroCenter provides the best possible treatment and management of neurological conditions including back and neck pain, headaches, neuromusculoskeletal disorders, dizziness, and neurological diseases.
At the NeuroCenter, we keep pace with the latest information in the field of neurology in order to offer the most effective programs for managing pain, headache and neurological disorders.
Appointment Information
Welcome to the NeuroCenter! We want you to feel at home in our professional, friendly, and caring atmosphere. The following is important information, so that you understand the way the NeuroCenter works for you--the most important person in the Center.
Your Initial Telephone Call
Please call us anytime at 800-396-3966. If you call during office hours someone will tend to your call and answer any questions you may have about our physician and services. If you call when the office is closed, messages are taken by our 24-hour answering service. Those messages are responded to promptly on the next business day.
We will not make you wait weeks or months to get help. We will try to schedule you right away!
What to Expect During Your Initial Visit
Plan to spend two hours in the office. You will fill out a thorough medical questionnaire. It is very important for the neurologist to have comprehensive information about your health. Ask our staff if you need any assistance --- we are happy to help you with any questions you may have.
We pride ourselves in listening to our patients carefully and are responsive and receptive to questions or suggestions that you may have. We keep a friendly and caring attitude and have a keen awareness that we are dealing with a human being--not just a disease or a medical condition.
Telephone Consultations
During the course of your treatment, you may occasionally need a telephone consultation with the doctor. You may call any time 1-773-775-7540 and leave a message with our staff for your specialist. Messages are answered within 24 hours except on weekends and holidays. Please note when the office is closed, messages are taken by our 24-hour answering service. Those messages are responded to promptly on the next business day.
Medication Refills
During your visit, Doctor will assure that you have enough medication until your next office visit. However, under unusual circumstances, you may anticipate running out before the next appointment. In that case, call your pharmacist and ask him/her to fax the prescription refill request to 1-773-763-9792. The prescription will be approved and faxed back to the pharmacy within 24 hours with the exception of weekends and holidays.
Please note, a refill request can only be approved for patients who have an upcoming appointment and have been seen recently.
Narcotic medications cannot be refilled on the telephone.
Narcotic medications or their prescriptions cannot be replaced if lost for any reason.
Prescription of a new medication that our Doctor has never prescribed usually requires an office visit.
Canceling Your Appointment
If you are unable to keep your appointment, we appreciate notification of at least 24 hours before your scheduled appointment. Our office personnel will be happy to reschedule your appointment. Please call 1-773-775-7540.
Emergencies
Emergencies are serious problems that cannot be treated over the telephone. If you have an emergency, call 1-773-334-5709 at any time. If the emergency occurs during office hours, you will be told to come to the office unless the emergency is of such nature that it cannot be handled in the office. In that case, you will be instructed to go to the Emergency Room of any hospital. If the emergency occurs during any other time, you will be told to go to the Emergency Room. If possible we will prefer you to go to the Emergency Room at Methodist Hospital of Chicago 1-773-271-9040 or Swedish Covenant Hospital 1-773-878-8200. During extreme emergencies you will be taken to the nearest Emergency Room. The Emergency Room physician will contact our doctor.

We accept all PPO Insurances. If you have a question about your specific plan, please call at 1-800-396-3966. We would be glad to look into your insurance plan specifically.

Financial Policy
Co-payments
- All co-pays are due, in full, at the time of service, per your insurance contract.
- We accept Visa, Mastercard, and American Express
- Any charges that have been cleared by your insurance plan (i.e. deductibles, patient percentage, services not covered by your plan) are due upon billing and may be paid at the office.
- Patients with balances older than ninety (90) days will be referred to the business office staff or the operations manager.
